Can I switch to Nebilet gradually from Concor cor?

I am currently taking Concor cor 1.25, mg (1 tablet in the morning, half a tablet in the evening). The doctor recommended changing to Nebilet 5mg (half a tablet in the morning, half in the evening). Can I switch to Nebilet gradually from Concor cor? I once tried to stop this small dose of Concor cor and unfortunately I had chills, a feeling of cold, etc.

Concor Cor i Nebilet to leki wydawane na receptę - zatem o sposobie przechodzenia z jednego leku na drugi może zadecydować wyłącznie lekarz zapoznany z Pani stanem zdrowia i wartościami ciśnienia.

Osoby postronne, np. farmaceuta nie mają takiej wiedzy, zatem nie są w stanie dobrać odpowiedniego dawkowania.
