In the current guidelines for the treatment of depressive and anxiety disorders, the combination of moclobemide and opipramol is not a standard therapeutic regimen. Usually, one antidepressant is used, and the possible combination of drugs is based on well-researched and safe combinations.
Moclobemide (MAO-A inhibitor) is a drug with a specific action profile that requires caution when combining with other substances that act on the serotonin system. Opipramol, on the other hand, has anxiolytic and sedative effects, but it is not a classic antidepressant.
While this combination may theoretically be considered on a case-by-case basis, it is not routinely recommended in guidelines and requires close medical scrutiny due to the potential for interactions referred to as serotonin syndrome. Serotonin syndrome is a dangerous reaction of the body to too high levels of serotonin, often caused by the combination of certain medications.
Typical symptoms include: fever, sweating, rapid heartbeat, anxiety, agitation, disorientation, muscle tremors, stiffness, difficulty coordinating movements, strong reflexes, sometimes muscle cramps.
If the treatment does not work, he or she may consider a change of strategy in consultation with a doctor – e.g. replacing opipramol with another antidepressant with a stronger activating effect.
