Can you take 4 tablets of Medikinet 20 mg per day?

My friend has been taking medicinette 20 for several years now and claims that the doctor recommended that he take four tablets a day, I'm afraid that this is already abuse because he started to behave irrationally, I noticed enlarged pupils and nervousness, is it possible for the doctor to prescribe such a dose
