Abdominal pain after consuming lemon during treatment with Pylera may be caused by irritation of the stomach mucosa. Pylera, used to treat Helicobacter pylori infections, contains substances that can increase the stomach sensitivity to acids, and lemon is a powerful acid. For this reason, combining the two can lead to discomfort.
It is also worth remembering that Pylera can cause side effects such as abdominal pain or nausea. If the pain is intense or persists, it is recommended to consult a doctor to assess the situation and possibly modify the diet or therapy.

You can eat a salad with lemon juice, as long as it does not contain dairy ingredients (e.g. cheese, cream).
For more information:
"The absorption of tetracycline is slowed down by antacids containing aluminum, calcium or magnesium, preparations containing iron, zinc or sodium bicarbonate, as well as dairy products. Therefore, these products should not be used concomitantly with Pylera."
