Such a swap may not be possible.
In the case of modified-release drugs, such as Medikinet CR, it should not be assumed that 1+1=2. Administration of two tablets instead of one may result in a different pharmacokinetic profile and thus a different therapeutic efficacy and safety profile.
A new prescription will be necessary.
