Can such a large hematoma and bruising of the hand after the introduction of umplant affect its effectiveness? Doesn't it block the substance that is supposed to spread through the bloodstream? The implant was replaced with a new one.

You should relieve your hand as much as possible until the pain stops.
The effectiveness of the implant is maintained.
Since yesterday my shoulder blade hurts and the place where I have the contraceptive implant hurts, I don't know if I overloaded it with something or if I was blown by the fan, does it have any effect on the contraceptive implant?

Bruising (in some cases very severe), pain and swelling after removal and insertion of the Implanon NXT implant are normal and common symptoms. This is not a cause for concern and does not affect the contraceptive efficacy of the implant. If you experience any other worrying symptoms, such as fever, worsening pain, redness or malaise, contact your doctor.
