The doctor prescribed medication for you on the so-called e-prescription. To redeem it, you need to go to the pharmacy, enter the four-digit code written at the top (1642) and your PESEL number. As far as I can see, the prescription should contain an antibiotic (clindamycin in a dose of 600 mg) and (most likely, a blurred spelling) the painkiller Nimesil. After providing the required data, it will be possible to buy medicines at the pharmacy. Please note that in Poland a prescription for an antibiotic is valid only for 7 days.

1x8 probably means taking 1 tablet/sachet every 8 hours, while 1x12 - taking 1 tablet every 12 hours. If you have any doubts about the dosage, I encourage you to consult your doctor.
