4 days ago I took 1 sachet of nimesil for a headache, after 4 days I had a bad headache again so I took 1 sachet of nimesil again.

Nimesil (and all non-steroidal anti-inflammatory drugs) can cause irritation of the gastrointestinal tract. In this case, the drug should be discontinued or replaced with another preparation (e.g. a paracetamol-based analgesic). It is worth informing your doctor about the observed side effects to avoid a similar situation in the future.

"Bleeding, ulceration, or perforation of the gastrointestinal tract may occur at any time during treatment with or without a history of warning symptoms or gastrointestinal events.
If gastrointestinal bleeding or ulceration occurs, nimesulide should be discontinued immediately. Nimesulide should be administered with caution to patients with gastrointestinal disorders, including a history of gastric ulcers, gastrointestinal bleeding, ulcerative colitis or Crohn's disease."
