The potency of opioids is calculated using oral morphine equivalents (OME).
Fentanyl patches are much more potent than morphine, with a dose of 25 μg/h equivalent to approximately 60–90 mg of oral morphine per day, and 50 μg/h equivalent to 120–180 mg. Doreta (tramadol + paracetamol) has a weaker effect than opioids.
Fentanyl, while effective and better tolerated by patients with gastric problems, is associated with a higher risk of respiratory depression and addiction. The choice of therapy should take into account efficacy, tolerability, and the risk of side effects.
