A properly issued prescription for Azopt drops should include the details of the facility, patient data, number of packages, frequency and method of use of the drug, payment, and the details of the doctor issuing the prescription. If the doctor has not specified the method of use of the drug or determines it incorrectly, a maximum of 4 packages of drops can be dispensed. If the patient has additional qualifications, the doctor should include them on the prescription.
