How to use Momester in a 3-year-old?

Good morning

My 3-year-old son is prescribed Momester 2 x 1 dog. Unfortunately, I didn't ask the doctor if this means that he should get 1 spray in each hole twice a day, or 1 spray in 1 hole twice a day. Please answer. Best regards.
