Azathioprine, being an antimetabolite drug, can theoretically interfere with the metabolic activity of hair follicles. Scientific papers describe cases of hair loss in the anagen phase (active phase of hair growth) associated with the use of this substance.
Alopecia during azathioprine therapy is described as a rare side effect, i.e. in ≥ 1/10000 to <1/1000 of people taking the drug.
In many cases where patients who experienced this side effect spontaneously resolved alopecia during continued treatment.
