Yes, if the intercourse is unprotected, fertilization may occur. The drug may cause reduced fertility (in both women and men), but it does not exclude the possibility of pregnancy. Due to the toxic effect on embryo development, pregnancy is an absolute contraindication to the use of methotrexate, and an effective method of contraception must be used throughout the treatment period.

"Pregnancy should be excluded before starting Metex in women. Methotrexate causes embryotoxic effects in humans, causes abortions and birth defects. Methotrexate affects spermatogenesis and oogenesis and can reduce fertility. These side effects appear to be reversible upon discontinuation of treatment. During treatment and for at least six months after treatment, women and men should use effective contraceptive methods."
"Metex is contraindicated in pregnant women. Animal studies have shown reproductive toxicity with methotrexate.
Methotrexate has been shown to be teratogenic in humans: still-abortions and birth defects have been reported. After exposure, a small number of women showed an increase in the incidence of congenital foetal malformations (skull, cardiovascular and limb defects). Normal pregnancy has been reported if methotrexate is discontinued prior to conception. During treatment, women should avoid pregnancy. If a woman becomes pregnant during treatment, medical counseling should be provided regarding the risk of methotrexate side effects on the child. Therefore, patients of both sexes must use effective contraceptives during treatment with Metex and for at least 6 months after puberty has reached puberty.
In women of childbearing potential, pregnancy should be excluded beyond reasonable doubt, e.g. by performing a pregnancy test before starting treatment.
Since methotrexate is genotoxic, a consultation with a genetic counselling centre should be recommended prior to initiation of treatment by women planning pregnancy. Men should seek advice on sperm deposit before starting therapy. "
