Here you can find a list of pharmacies in Warsaw that have the dengue vaccine. In some pharmacies, you can get a pharmaceutical prescription and take the vaccine right away (information that the pharmacy provides such a service can be found next to the name of the facility):

Vaccination can be performed at the Provincial Infectious Diseases Hospital at 37 Wolska Street. The price list available on the hospital's website gives the amount of PLN 700 for vaccination:
