Some pharmacies provide a service of issuing a pharmaceutical prescription for the vaccine and administering the vaccine. The information is contained next to the name of a specific pharmacy. It should be borne in mind that then there is no discount on the vaccine, as a pharmaceutical prescription can only be issued with a 100% payment. The price is then PLN 751.16
However, as announced by the Ministry of Health, this is soon to change and the pharmacist will be able to issue a prescription for this drug at a discount, including free of charge for people 65+ (as in the case of pneumococcal vaccines).
At the moment, to receive the vaccine at a discount (and free for seniors), a prescription from a doctor is necessary.
The discount is available to people aged 18 and over with an increased risk of developing shingles, i.e. with: chronic heart disease, chronic lung disease, diabetes, chronic kidney failure, congenital or acquired immunodeficiency, generalized cancer, HIV infection, Hodgkin's disease, iatrogenic immunosuppression, leukemia, etc. Then the price for 1 dose of the vaccine is PLN 375.58.

As a pharmacist, I can only give you general information. In general, shingles vaccinations are usually recommended for people who have had shingles in the past, even if they have already had it. However, before you decide to get vaccinated against shingles (herpes zoster), you should consult your doctor about it.
People in their 75s may be particularly susceptible to complications from shingles, so it's worth discussing the fact that you've had it before with your doctor. If you do not have the possibility of an in-person visit, I encourage you to take advantage of the teleconsultation, e.g. via the Gdziepolek.pl portal:
